Case Manager

Job Title: Case Manager Part Time/Full Time starting at 15 hours per week

Reports to: Program Manager

Qualifications: Bachelor’s Degree in Human Services or closely related field. Excellent interpersonal and group communication skills, organizational skills, writing skills, time management and computer skills are all required. A minimum of two years working with youth. Must be flexible. Candidates MUST have a current CT Driver’s License, current automobile insurance and a reliable vehicle. Covid vaccination required. Bilingual or Multilingual (English/Spanish/other) is a plus but not required.

Employment is contingent on successfully clearing a criminal background, DCF, and police checks and all pre-hire requirements.

Duties include but are not limited to:

• Provide 15-40 hours of case management services
• Teach and reinforce all life skills documented on the Life Skills Inventory, i.e. Budgeting, Housing, Education, and Employment
• Be knowledgeable of community resources and provide information and assistance with community linkages for development of natural supports
• Assist clients with finding and enrolling in suitable secondary education and obtaining financial aid
• Monitor educational status including attendance and grades
• Assist clients with resume writing and job searches
• Assist new clients and those wishing to move with locating and maintaining housing
• Be knowledgeable about program guidelines and DCF policies and procedures
• Ensure documentation is completed as required: monthly and weekly reports, updates, weekly calendar, and case notes.
• Be available by phone to receive client information for emergencies
• Participate in all meetings as determined by the Program Director
• Maintain a flexible schedule based on program needs
• Any other duties as assigned by the Program Director

Positive Youth Development Team Leader

Job Title: Positive Youth Development Team Leader

Reports To: Director of Program Operations

Qualifications: Excellent communication, leadership and organizational skills. Relevant past experience in the social service and non-profit field. BS in a related field. Experience and training in high risk youth and family issues. Public speaking and experience in training.

Team Leader brings the team together to strengthen existing programs and formulate new opportunities for programs and funding sources.

Employment is contingent on successfully clearing a criminal background, DCF, and police checks and all pre-hire requirements.

Responsibilities:

• Oversight of team including monthly program staff meetings and individual meetings with staff as needed to keep updated on programs.
• Manage Positive Youth Development programs such as, mentoring, after school programming, and LGBTQ+ Support, including staff supervision, training, and evaluations.
• Responsible for maintaining and collecting data for outcome measurements.
• Timely submission of reports.
• Assist with grant preparation.
• Coordinate training for team when appropriate.
• Recommend staff on appropriate task forces, coalitions, etc.
• Perform community outreach to area agencies and providers as requested to do so and other duties as required.
• Convey the concerns, attitudes and issues of staff to the Director of Program Operations.
• Contribute to Agency cohesiveness by maintaining a collaborative and team oriented relationship.
• To maintain a role in positively representing the Agency its mission and services to the community.
• Complete auditing and surveys for programs and program participants.
• Work with executive team to reach program goals and objectives for funders.
• Oversee agency events as needed.
• Assist with recruiting.
• Assist with reports to funders.
• Take an active role with data collection.

Pay: $50,000.00 - $55,000.00 per year

Assistant Out of School Facilitator/Driver

Job Title: Assistant Out of School Suspension Facilitator/Driver

Reports To: Out of School Suspension Supervisor

Qualifications: Excellent driving record. Spanish-speaking is a plus.

Employment is contingent on successfully clearing a criminal background, DCF, and police checks and all pre-hire requirements.

Responsibilities:

• Transport up to ten high school youth in agency vehicle to and from program each day Monday-Friday during the academic year.
• Establish and maintain appropriate rapport with students.
• Monitor daily attendance and report information to facilitator.
• Monitor student's behavior while in program.
• V endorsement not required but preferred.
• Monitor student behavior while in the vehicle to ensure a safe environment for all.
• Plan the ideal transportation route each day depending on the location of each youth’s home.
• Ensure maintenance on the van is completed in a timely manner.

Pay: $18.50 per hour, 8am-1pm

Out Of School Suspension Facilitator

Job Title: Out of School Suspension Facilitator

Reports To: Out of School Suspension Supervisor

Qualifications: Bachelor’s Degree, experience teaching students in High School (grades 9-12), with a focus on Math, the Sciences, and/or Language Arts (English and Reading). Excellent interpersonal communication skills, organizational and writing skills, time management and computer skills are required. Spanish-speaking is a plus.

Responsibilities:

•Facilitate out of school suspension program. The program is conducted Monday-Friday during the academic year from 7:30am-2:30pm.
• Provide classroom structure throughout the school day for 10 high school youth referred to Reboot out of school suspension program.
• Monitor completion of student assignments through their Chromebooks.
• Complete all required progress reports for Waterbury Public Schools.
• Work with Waterbury Public Schools to document progress in Out of School Suspension program and facilitate a smooth return to school at the end of the suspension period.
